SightMD Performs First Cataract Surgery Using Apple Vision Pro Mixed Reality Platform

Breakthrough procedure highlights convergence of spatial computing and ophthalmic surgery, enabling immersive visualization and real-time collaboration

SightMD has announced a major milestone in surgical innovation, with Dr. Eric Rosenberg becoming the first surgeon in the world to successfully perform cataract surgery using the Apple Vision Pro mixed reality platform.

The procedure, initially completed in October 2025, has since been followed by hundreds of additional cases, demonstrating both the scalability and clinical applicability of the technology in real-world surgical settings.

The system is powered by ScopeXR, a spatial computing platform co-developed by Dr. Rosenberg, which integrates real-time surgical imaging, preoperative diagnostic data, and digital overlays directly into a head-mounted display. This allows surgeons to visualise the operative field in immersive 3D while maintaining sterile technique, marking a departure from traditional microscope-based workflows.

Unlike conventional surgical setups, the mixed reality platform enables simultaneous access to multiple layers of information without requiring surgeons to shift focus away from the procedure. The system is also compatible with existing 3D digital surgical microscopes, supporting easier integration into current operating room infrastructure.

Making use of the real-time collaboration ability, surgeons and experts can join procedures virtually, viewing the same surgical field and data in real time, while communicating through integrated audio systems. This has significant advantages for training, mentorship, and intraoperative decision support, particularly in complex or high-risk cases.

Beyond clinical use, the platform is expected to transform surgical education by enabling medical trainees to observe procedures remotely with enhanced clarity and immersion, reducing the need for physical presence in operating rooms.
